First-years and Transfers:

• Step 1 (Must be completed before try-out)
  • Create a SportsWare account 
• Step 2 (Must be completed before try-out)
  • Complete all information on SportsWare: demographic information, emergency contact, insurance information and card, and health history
• Step 3 (Must be completed before try-out)
  • Upload a physical examination dated no more than 6 months before the first day of participation to the Attachments page on SportsWare 
• Step 4 (Must be completed before first competition)
  • Upload a copy of the Sickle Cell Trait test lab results or Newborn HgbAS screening result to the Attachments page on SportsWare 
• Step 5 (Must be completed before try-out)
  • Concussion Baseline Test on SWAY Medical app 

Returning Athletes:

• Step 1 (Must be completed before try-out)
  • Review your SportsWare information and make any necessary changes, (contact information, insurance, etc.)
• Step 2 (Must be completed before try-out)
  • Review and update full health history on SportsWare and with physician during pre-participation physical examination
• Step 3 (Must be completed before try-out)
  • Upload a physical examination dated no more than1 year before the first day of participation to the Attachments page on SportsWare 
• Step 4 (Must be completed before try-out)
  • Concussion Baseline Test on SWAY Medical app
