Mission Statement
The Purchase College Sports Medicine Department is committed to providing the utmost comprehensive healthcare available to our student-athletes through prevention, evaluation, management, and rehabilitation of injuries. Our objective is to address the healthcare needs of each individual student-athlete while striving to return them to competition as quickly and safely as possible.

First-years and Transfers:
- Step 1 (Must be completed before try-out)
- Step 2 (Must be completed before try-out)
- Complete all information on SportsWare: demographic information, emergency contact, insurance information and card, and health history
- Step 3 (Must be completed before try-out)
- Upload a physical examination (link) dated no more than 6 months before the first day of participation to the Attachments page on SportsWare
- Step 4 (Test results must be submitted before first competition)
- Upload a copy of the Sickle Cell Trait (link) test lab results or Newborn HgbAS screening result to the Attachments page on SportsWare
- Step 5 (Must be completed before try-out)
- Concussion Baseline Test on SWAY (link) Medical app
Returning Athletes:
- Step 1 (Must be completed before try-out)
- Review your SportsWare (link) information and make any necessary changes, (contact information, insurance, etc.)
- Step 2 (Must be completed before try-out)
- Review and update full health history on SportsWare and with physician during pre-participation physical examination
- Step 3 (Must be completed before try-out)
- Upload a physical examination (link) dated no more than1 year before the first day of participation to the Attachments page on SportsWare
- Step 4 (Test results must be submitted before first competition)
- Concussion Baseline Test on SWAY (link) Medical app
