Required Compliance Forms

Hello Purchase College Athletes!

My name is Francesca Levine, and I am the Associate Director of Athletics/Senior Woman Administrator here at Purchase. I am also responsible for student-athlete compliance and eligibility. Everything listed below MUST be completed prior to practice and competition.

The attachments labeled “TO READ” includes all of the information that you as a student-athlete are responsible for. Please complete each of the 3 steps listed below to ensure that you are ready to practice and compete with your team!

Purchase College Student-Athlete NCAA Compliance Eligibility Checklist

STEP 1: Complete Purchase College Athletics Required Forms Using FORMSTACK
- Division III summary of regulations document
- Purchase College Conduct Statement
- Banned Substance List
- Purchase College Required Forms: Click HERE to complete them online!

STEP 2: Complete NCAA Required Forms
- All athletes will receive an email from forms@ncaa.org
- Follow instructions to log in and complete/review the following forms:
  - General Statement Concerning Eligibility
  - FERPA/HIPAA Consent
  - Amateurism
  - Drug Tests- YOU MUST AGREE AND ANSWER “YES” TO DRUG TESTING CONSENT FORM
  - Sports Wagering
  - Academic Eligibility Information (Freshman Only)
  - Other Prior Violations
  - Information Pertaining to Future Transfer
  - Termination/Survivability of Student-Athlete Statement
  - Student-Athlete Signature

STEP 3: Purchase College VECTOR/EverFi Required Educational Course Assignment: Sexual Assault Prevention for Athletes
- All athletes will receive an email message from the college with the subject “EverFi Required Educational Course Assignment”
- All athletes will receive this course through their EVERFI account. To access your account click HERE
- Your EverFi account is the same username/password as your Purchase email
- All athletes must complete this course each year, prior to their first practice
- THE NAME OF THE COURSE IS: SEXUAL ASSAULT PREVENTION FOR ATHLETES
